Find your NHS Number
The registration form will ask you for your NHS number which consists of a 10 digits. You can find it on a prescription, a clinical letter or by using Find your NHS number - NHS (www.nhs.uk)
Using an online form
You can check if you're eligible and register with the practice using an online form.
The registration process takes around 15 minutes to complete.
Register with a GP surgery (gp-registration.nhs.uk)
Using a paper form
You can register with the practice by filling in a paper form.
You can collect a registration pack from the surgery, Its helpful if you bring some photo ID and proof of address within the last 6 months.
